dc.description.abstract |
The paper describes a case study of a community health model that was developed and piloted at a University based clinic
run in partnership with the South African Department of Health (DOH). The aim was to strategize, innovate and implement a
collaborative care model strengthened by available technological resources that could improve the health and social
outcomes of a struggling community.
The 5 C’s e-Health framework for developing countries was applied to the case and compared to the Human Resources for
Health 2030 strategy of Government to inform the development of a community care team (human capital based and
enabled by an electronic management system) that could share patient health and social information, synergise resources
and inform future community developments. |
