The aim was to strategize, innovate and implement a collaborative care model strengthened by available technological resources that could improve the health and social outcomes of a struggling community. The 5 C’s e-Health framework for developing countries was applied to the case and compared to the Human Resources for Health 2030 strategy of Government to inform the development of a community care team (human capital based and enabled by an electronic management system) that could share patient health and social information, synergise resources and inform future community developments.enJournal of Contemporary ManagementCommunity healthElectronic health systemNational health insurancePrimary health care (PHC)Technological health developmentsTechnology as a tool to develop a community health modelArticle
