Chronic rheumatic valvulitis of aortic valve with infective endocarditis and perforation of one valve cusp.2. Secondary incompetence of mitral- and tricuspid valves.3. Hypertrophy and dilatates of both ventricles.4. Chronic venous congestion.Diagnosis: Chronic rheumatic valvulitis of aortic valve and infective endocarditis with cusp perforation
