Clinical history : A 49 year old male with a atrio-septal infarction and 2 years later an inferior infarction was found and later reported an atrio-septal infarction with a bilateral bloackage.
The patient then had an attck as a result of left ventricular fibrosis and mitral incompetence.
The left mamillary artery was implanted in the left ventricular myocardium and the patient experienced pronounced mitral insufficency and right pleural effusion after the operation.
Autopsy findings : 1. 3 areas of old myocardial infarction.
2. Old fibrosis and necrosis of the papillary of the anterior mitral valve segment in the area of the mitral incompetence.
3. The mamillary artery implanted in the left ventricular myocardium.
4. chronic venous congestion.
Diagnosis: Coronary atherosclerosis and thrombosis with three old myocardial infarctions and attack of the papillary muscles with mitral incompetence
