Various structural anomalies of the papillary muscles have been described in a variety of primary and secondary cardiovascular disorders. Some of these lead to intraventricular pressure gradients, while some has no obvious functional consequences at present. A peculiar anterolateral papillary muscle anomaly with an accessory papillary muscle, causing the appearance of a mirror image on transthoracic echocardiography is described.en_US
